Biopsychosocial Assessment Tools in Practice: Comparing Standardized Instruments Against

Sylviastella Favour Peteranaba, Helen Ekwi Osinem, Florence Eribenne, Olakunle, Saheed Soyege

Abstract

This review critically examines how standardized instruments and individualized clinical evaluation contribute to biopsychosocial assessment across diverse practice settings. Its purpose was to determine how each approach addresses multidimensional clinical needs, including biological, psychological, social, functional, cultural, and contextual factors, while considering reliability, validity, clinical utility, person-centeredness, and decision-making value. A structured narrative review approach was adopted, drawing on theoretical, psychometric, clinical, cross- cultural, and implementation literature published through 2023. The evidence indicates that standardized tools strengthen consistency, transparency, screening accuracy, severity estimation, functional measurement, and longitudinal monitoring, particularly when instruments are locally validated and linked to explicit clinical decisions. However, standardized scores may underrepresent culturally specific meanings, contextual influences, complex life histories, and atypical presentations. Individualized interviews and professional judgment address these limitations by eliciting narrative, relational, cultural, and situational information, although unaided judgment remains susceptible to bias and inter-clinician variability. The review therefore finds that neither approach is sufficient when used in isolation. The most defensible model combines validated standardized measures with structured, culturally responsive interviewing and multidisciplinary interpretation. Such integration enables reproducible measurement while preserving individualized meaning, contextual precision, and therapeutic relevance. The review recommends greater investment in cross-cultural validation, locally grounded instrument development, implementation research, shared clinical formulations, and comparative studies of integrated versus single-method assessment models. Future research should prioritize diagnostic accuracy, patient experience, treatment modification, equity, feasibility, cost, and longitudinal outcomes. Future scholarship should also evaluate digital tools without displacing clinical interpretation. Overall, integrated biopsychosocial assessment offers the strongest basis for scientifically rigorous, culturally sensitive, and clinically meaningful decision-making across complex and diverse care environments.
