The Role of Medicine Quality Failures in the Emergence and Spread of Antimicrobial Resistance: A Narrative Review
Abstract
Antimicrobial resistance is among the most serious threats to global health, yet its drivers extend beyond the inappropriate use of antimicrobials that dominates most policy discussion. The quality of the medicines that patients actually receive is a frequently overlooked factor. Substandard and falsified antimicrobials, the two principal forms of medicine quality failure, circulate widely in low- and middle-income countries and expose pathogen populations to active pharmaceutical ingredient at concentrations that differ from the intended dose. This narrative review synthesises the conceptual, mechanistic, and empirical literature on whether such quality failures contribute to the emergence and spread of resistance, and it situates that question within the wider epidemiology of resistance, the pharmacology of subtherapeutic exposure, the analytical science of medicine quality, and the international regulatory response. Poor-quality antimicrobials can plausibly accelerate resistance by exposing organisms to subtherapeutic concentrations within the mutant selection window, by sustaining low-level selective pressure that favours resistant variants and their compensatory evolution, by introducing cryptic or incorrect active ingredients, and by undermining the combination therapies designed to suppress resistance. The relationship is not, however, unidirectional, because depending on the dose actually delivered, poor-quality products may in some circumstances reduce rather than increase selection pressure, which leaves the net population-level effect uncertain. Direct evidence linking medicine quality to measured resistance remains sparse and methodologically heterogeneous, and the surveillance systems for resistance and for medicine quality have largely developed in isolation from one another. Medicine quality failure is therefore a biologically plausible and neglected contributor to AMR that warrants explicit inclusion in national action plans, alongside strengthened quality assurance, field-deployable detection, linked surveillance, and dedicated research designed to quantify its magnitude.
