A Conceptual Framework Review of Incentive Alignment Mechanisms Between Payers and Providers in Value-Based Care Contracting and Their Effects on Cost and Quality Outcomes

Abstract

Value-based care contracting has become the dominant policy vehicle for reorienting health systems away from volume-driven payment and toward payment that rewards efficiency, clinical quality, and population health improvement. At the center of every value-based arrangement sits a set of incentive alignment mechanisms, which are the contractual instruments through which payers attempt to make the financial interests of providers converge with the goals of cost containment and quality improvement. Despite two decades of experimentation, the literature remains fragmented across payment models, care settings, and evaluation designs, and no unified conceptual account explains why apparently similar mechanisms produce divergent results. This paper develops a conceptual framework that organizes incentive alignment mechanisms along five structural dimensions: the locus and magnitude of financial risk, the architecture of performance measurement, the unit of accountability, the benchmarking and attribution logic, and the temporal structure of reward. The framework links these design dimensions to cost and quality outcomes through mediating pathways of provider behavioral response and organizational care redesign, and it specifies moderating conditions, including organizational capacity, market structure, patient population characteristics, and data infrastructure, that condition whether alignment translates into performance. Drawing on theoretical foundations in agency theory, multitask incentive theory, transaction cost economics, and behavioral economics, and on empirical evidence from pay for performance programs, accountable care arrangements, bundled payment initiatives, and global budget contracts, the review synthesizes what is known about the cost and quality effects of each mechanism class. The framework clarifies persistent design tensions, identifies conditions under which incentive alignment succeeds or fails, and sets out an agenda for research and contract design practice.
